A prospective population-based cohort study examined for the first time the relationship between early childhood systemic inflammatory response and the development of molar incisor hypomineralisation (MIH) — a common defect that significantly affects the quality of life of children, yet its aetiology remains unclear.
Australian researchers from Melbourne Dental School conducted an analysis of data from the Barwon Infant Study population study, which tracked the development of participants over time. Scientists measured two key markers of systemic inflammation in maternal serum during pregnancy, and then in children’s blood at birth and at ages 6, 12 months and 4 years.
Unexpected study result
Contrary to expectations, the study found no association between levels of inflammatory markers in early childhood and the development of MIH. Children who subsequently received an MIH diagnosis demonstrated comparable concentrations of inflammatory proteins with children in whom the defect did not develop — in both mild and severe forms of the disease.
«We were surprised to find that systemic levels of inflammation markers are not associated with MIH,» noted Dr Mihiri Silva, senior author of the study and associate professor of paediatric dentistry.
Given the prevalence of MIH (approximately 14% of the global population) and its significant impact on the dental status of children — patients with MIH receive 4.8 times more dental interventions by age 18 — the search for causes remains a priority.
New research directions and multifactorial aetiology
The findings indicate a multifactorial aetiology of MIH involving genetic and environmental factors. Researchers suggest that other forms of inflammation associated with atopic conditions — asthma and eczema — may play a role.
The authors emphasise the need for large-scale studies with standardised methodology to identify other potential risk factors. The study was published on 22 July 2026 in Journal of Dental Research.
