Omega-3 Fatty Acids in Periodontal Therapy: Tufts University and a New Paradigm of Dietary Interventions in the Management of Oral Inflammation

A study conducted at Tufts University has expanded understanding of the link between periodontitis and cardiovascular disease, shifting focus toward dietary interventions as a possible complement to mechanical oral sanitation. A team led by Dr Bushra Ahmad, Associate Professor of Periodontics at Boston School of Dental Medicine, investigated the effects of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) — key components of omega-3 — on inflammatory processes both in the oral cavity and in systemic circulation.

Although the anti-inflammatory properties of omega-3 are well-known, their specific effects on periodontal inflammatory markers in humans remained insufficiently studied. This made the topic relevant for clarifying the possibility of systemic dietary intervention complementing traditional approaches to managing oral inflammation.

Study Design and Methodology

The study included 240 patients with stable coronary artery disease receiving statin therapy. Over 30 months, half received a high-dose prescription drug containing EPA and DHA, while the control group received no supplements.

Gingival fluid samples were collected from maxillary quadrants. Researchers compared inflammatory markers in gingival fluid and systemic inflammation markers in blood between groups.

Results and Conclusions

The omega-3 group demonstrated a significant reduction in one of the periodontal inflammation markers, predominantly in the left quadrant. These patients also exhibited a lower neutrophil-to-lymphocyte ratio — a marker of systemic inflammation associated with cardiovascular outcomes.

Dr Ahmad suggested that lateral differences may be related to individual oral hygiene practices and dominant hand: right-handed individuals likely more effectively remove dental plaque from the left side. However, the authors emphasized that this assumption requires further verification.

Context and Perspectives

The findings suggest a potential role of omega-3 as a supplement to mechanical plaque removal in reducing periodontal and systemic inflammation. The study is part of a larger project examining links between periodontal and systemic pathology, including effects on cardiac health and cognitive function.

Dr Ahmad noted that the results should not be extrapolated to other patient populations or other forms of omega-3 supplements. Additional research is needed to assess long-term periodontal and cardiovascular benefits in broader population groups.
