Although certification is not mandatory, the council recommends physicians engaged in the removal of third molars to undergo specialized training beyond basic preparation and to work exclusively within the scope of their competence.<\/p>\n<h2>Requirements of the accreditation program<\/h2>\n<p>Dentists who have completed an SDC-approved course may apply for an initial three-year accreditation. The program must be completed within six months and includes <strong>a minimum of 8 hours of theoretical training, observation of at least one operation, and no fewer than five supervised clinical cases<\/strong>, assessed as satisfactory. To obtain permanent accreditation after the initial period, physicians must provide evidence of performing a minimum of 30 third molar extractions over these three years.<\/p>\n<p>The framework also defines which cases of mandibular third molars are considered suitable for training. <strong>More complex impactions<\/strong> \u2014 including those with deep retention or proximity to the mandibular canal \u2014 are outside the scope of the program and are recommended for treatment by oral and maxillofacial surgeons.<\/p>\n<h2>Criticism of the framework in the professional community<\/h2>\n<p><strong>The Singapore Dental Association<\/strong> supported the goal of enhancing patient safety; however, it criticized the restrictive nature of the framework. In a letter published in August, the association&#8217;s president Dr Eugene Tang noted that the exclusion of complex cases &#8220;creates a potential ceiling&#8221; for development, leaving physicians without &#8220;a defined pathway for professional growth through further education, mentorship, and supervised experience&#8221;.<\/p>\n<p>Tang emphasized the need for greater dialogue with professionals and warned against overemphasis on the volume of procedures. In his view, <strong>clinical experience is important, but professional development should also take into account proper case selection, clinical judgment, outcomes, and continuing education<\/strong>.<\/p>\n<h2>Position of regulators and development perspectives<\/h2>\n<p>The SDC responded that the threshold of 30 cases is only one aspect of the assessment. Accreditation also considers <strong>the quality and range of clinical experience, proper supervision, professional judgment, and compliance with practice standards<\/strong>. The council indicated that feedback from physicians and professional organizations will be used in further refinement of the system.<\/p>\n<p>Local media note a heterogeneity of positions among stakeholders: experienced physicians question how the system recognizes already existing qualifications, while leaders of dental clinics see in a uniform standard an advantage for developing physician confidence and reducing unwarranted referrals to specialists.
