However, individual studies did not confirm a significant association, including one that found no direct biological link between cannabis and periodontitis.<\/p>\n<p>Overall, the authors considered <strong>periodontal disease as the outcome most consistently associated with cannabis use<\/strong> and recommended accounting for cannabis history when assessing periodontal risk, particularly in frequent and long-term users.<\/p>\n<h2>Xerostomia and dental caries: ambiguous evidence<\/h2>\n<p>The review revealed possible associations with xerostomia and dental caries. Frequent cannabis use was associated with subjective complaints of dry mouth; however, objective indicators of salivation proved contradictory. Some studies reported greater caries intensity in cannabis consumers, but this association weakened or disappeared when confounding factors were considered.<\/p>\n<p>The authors suggested that <strong>behavioral factors \u2014 frequent consumption of sweet beverages and snacks, poor oral hygiene \u2014 along with socioeconomic factors, tobacco use, and access to dental care<\/strong> may better explain differences in dental status between cannabis consumers and non-consumers.<\/p>\n<h2>Oncology: contradictory results<\/h2>\n<p>Data on the association between cannabis and malignant neoplasms of the head and neck proved inconsistent. One study linked cannabis use to HPV-positive head and neck cancer, but not to HPV-negative disease; however, a later study did not confirm this.<\/p>\n<h2>Anesthetic aspects<\/h2>\n<p>Anesthetic requirements varied depending on the type. During deep sedation or general anesthesia, <strong>cannabis consumers may require higher doses of propofol, midazolam, ketamine, and fentanyl<\/strong>. The efficacy of local anesthesia did not differ significantly. The authors suggested considering cannabis history when planning deep sedation or general anesthesia.<\/p>\n<h2>Microbiological mechanism<\/h2>\n<p>One possible explanation for the association between periodontitis and cannabis is considered to be a change in the oral microbiome. <strong>It was established that cannabis use was associated with distinctive profiles of oral bacteria, with certain potentially pathogenic microorganisms demonstrating resistance to the antimicrobial action of cannabidiol<\/strong>, which could create favorable conditions for their selection.<\/p>\n<h2>Recommendations for clinical practice<\/h2>\n<p>The authors recommended that dentists regularly document cannabis use \u2014 including frequency, duration, route of administration, and time of last use \u2014 when evaluating patients and planning treatment.
