However, the expansion of technological capabilities inevitably necessitates a revision of the principles governing their use. In this context, the updated clinical guidelines from the American Dental Association (ADA), published in January 2026, reflect a significant shift toward a more balanced, evidence-based, and patient-centered approach to dental imaging.  <\/p><p class=\"wp-block-paragraph\">For the first time in over ten years, the ADA has presented comprehensive guidance that consolidates recommendations for patient selection for both two-dimensional and three-dimensional imaging methods. This document emphasizes that diagnostic imaging should not be prescribed based on templates or &#8220;just in case,&#8221; but strictly according to clinical indications, individual risk, and diagnostic value for the specific patient. <\/p><h3 class=\"wp-block-heading\">Clinical examination as the starting point of diagnosis<\/h3><p class=\"wp-block-paragraph\">The updated guidelines place particular emphasis on the fact that any decision to prescribe radiographic imaging should begin with a thorough clinical examination. This examination should include an analysis of the patient&#8217;s complaints, an assessment of their medical and dental history, and a review of previously taken images, if available. Imaging is viewed as an auxiliary tool for diagnosis and treatment planning, but not as a screening method for patients without clinical signs of disease.  <\/p><p class=\"wp-block-paragraph\">The ADA strongly recommends avoiding duplicate studies and performing imaging solely for convenience or routine practice. The frequency and type of imaging should be tailored individually, taking into account the patient&#8217;s age, risk level, presence or absence of pathology, and disease progression. <\/p><h3 class=\"wp-block-heading\">The role of two-dimensional radiography in everyday practice<\/h3><p class=\"wp-block-paragraph\">For diagnosing caries and endodontic diseases, intraoral two-dimensional radiography remains the primary imaging method. The recommendations specify the selection of projections based on anatomical features and suspected pathology, emphasizing that even with the availability of more advanced technologies, basic methods retain their clinical relevance. <\/p><p class=\"wp-block-paragraph\">Panoramic radiography is recommended as a primary method for orthodontic evaluation, assessment of dentofacial development, and for analyzing the position of third molars and supernumerary teeth when there are clinical grounds to do so. At the same time, it emphasizes the inadmissibility of routine panoramic screening of patients in the absence of indications. <\/p><p class=\"wp-block-paragraph\">In the field of periodontology, the ADA confirms that a comprehensive clinical examination combined with a full-mouth intraoral radiographic series provides an adequate baseline assessment of tissue condition. Subsequent use of imaging should be determined by disease progression and response to treatment, rather than fixed time intervals. <\/p><h3 class=\"wp-block-heading\">Limited and targeted application of CBCT<\/h3><p class=\"wp-block-paragraph\">According to the new guidelines, cone-beam computed tomography should be used exclusively in clearly defined clinical situations where additional anatomical information is needed that is not obtainable through methods with lower radiation exposure. Such situations include complex implant treatment planning, ambiguous diagnostic findings, re-treatment endodontics, and specific cases of traumatic injuries. <\/p><p class=\"wp-block-paragraph\">In all cases of CBCT use, clinicians are strongly advised to employ dose reduction strategies and carefully weigh the expected diagnostic or therapeutic benefit against the potential radiation risk to the patient.<\/p><h3 class=\"wp-block-heading\">Expert opinion and clinical philosophy<\/h3><p class=\"wp-block-paragraph\">Commenting on the publication of the guidelines, the lead author of the document, Dr. Erica Benavides, a clinical professor in the Department of Periodontics and Oral Medicine at the University of Michigan School of Dentistry, emphasized that diagnostic imaging can significantly improve both dental and general health outcomes, provided it is used appropriately. Together, these materials form a comprehensive approach to diagnosis, integrating clinical appropriateness, safety, and high standards of care. <\/p><h3 class=\"wp-block-heading\">Findings<\/h3><p class=\"wp-block-paragraph\">The updated ADA guidelines confirm dentistry&#8217;s shift toward a truly patient-centered diagnostic model. They provide clinicians with a structured framework for making decisions based on clinical evidence, individual risk, and the principles of responsible management of ionizing radiation sources.
